Tournament Entry Form
Entry forms must mailed with the appropriate fees directly to the tournament host. A separate entry form must be submitted for each tournament date and mailed to the tournament host. Click here to download the tournament entry form.
 


Team Roster
A copy of the completed team roster needs to be given to each tournament host at check in. The tournament host will keep this form. Do not mail this form to the to the tournament director. Click here to download a team roster form.

 

Tournament Check-in

Coaches will need either a 2005 permanent AAU Card or a 2005 temporary AAU card stamped in Red and Blue with MJVBA expires 2005 for every athlete & non-athlete.

In addition to the AAU cards and the team roster form coaches must have a copy of each players birth certificate so ages can be verified.

If you have athletes playing under the MJVBA age modifications for 14,13 or 12's check for the additional required paper work on the age definitions page.

 

Medical Authorization

All clubs should have on file a medical authorization slip signed by the athlete and a parent or guardian granting permission for emergency medical treatment. The form should list emergency contact information. A duplicate copy should be with the coach at every practice and competition.

 
 

Dexter Tournaments

Club Directors and coaches review the tournament check-in procedures listed in the column on the left. Team Rosters will be collected, AAU Cards & birth certificates reviewed.
